If you would like to make a referral for a psychoeducational assessment, please download the electronic form below and email the completed form to info@capitalsp.ca. 

 Please note

• At this time we are unable to accept referrals for one-on-one skills teaching/counseling due to a lengthy waitlist.
• New referrals are reviewed weekly and one of us will be in touch as soon as possible. 
• This form can be completed on a computer or mobile device.
